Before you come in for our first session, you will need to fill and sign the forms attached below.
1. Consent Form: This describes my office policies and important information regarding my practice including fees and cancellation policies. Please fill out the contact information and sign the form.
2. Acknowledgement of Receipt of Privacy Practices: This is the acknowledgment of having received the HIPAA privacy form. You can read and download the entire HIPAA Agreement at “HIPAA Notice of Privacy Practices.”
